HCAH Launches its First Transition Care Centre in Gurugram

 In line with its commitment to delivering hospital-like care beyond the four walls of the hospital, in over 70 cities across the country, HCAH (formerly HealthCare atHOME) has launched its third Transition care centre in India and first in North India in Sector-51, Gurugram. The Chief Guest, Dr. Yash Garg, IAS, Deputy Commissioner of Gururgram inaugurated the Centre.

“The way healthcare system has been unfolding, the demand for Transition Care has increased significantly in India. And, Gurugram, one of the fastest-growing cities of NCR, surrounded by a large population, has been in great need of Transition Care services. The Transition Care Centre launched by HCAH will cater to the growing demand, filling the massive healthcare continuum gap between tertiary care hospitals and homes,” said Chief Guest, Dr. Yash Garg, IAS, Deputy Commissioner – Gururgram.

The HCAH Transition Care Center is the perfect place where an experienced and multidisciplinary rehab care team in a state-of-the-art facility delivers holistic care. It ensures an early and effective recovery, helping patients return to their routine to the extent possible.

Sharing his thoughts on the Transition Care Center launch, Vivek Srivastava, Co-Founder & CEO – HCAH said, “Having assessed the growing need of Transition care, we have launched the centre in Gurugram to cater patients of Delhi NCR. The NCR region is an important hub for tertiary care for patients from India as well as abroad, which makes it an ideal spot for a transition care center to help people with clinical recovery and return to normal life. Many patients have recovered from a serious illness but are not well enough to go back home as they need a special kind of care and care environment. We hope this Transition care Centre will bridge the healthcare continuum gap between tertiary care hospitals and homes. “Transition care centres typically address care requirements of post-stroke, head and spine injury, geriatric, joint replacement surgery, early-stage cancer, post-transplant and other major post-surgical rehabilitative requirements through a multidisciplinary team comprise of doctors, physiatrists, physiotherapist, occupational therapist, clinical dieticians, and skilled nurses.  Without Transition care, there is an estimation that patients have a 30% higher chance of mortality, 30% higher chances of hospital readmission due to re-infections or disease recurrence, and 40% higher chances of preventable long-term disability. We hope that the newly launched Transition Centre in Gurugram, which has facilities like balance and gamification lab, mobility tracker, robotic hand, occupational therapy / ADL lab, and speech and swallow therapy to name a few, will meet the growing demand for Transition Care services,” says Dr. Gaurav Thukral, COO – HCAH.
