
Bhubaneshwar : Apollo Cancer Centre (ACC), Bhubaneswar, marked a major milestone in surgical oncology by successfully performing Odisha’s first Transanal Minimally Invasive Surgery (TAMIS). This groundbreaking procedure, led by Dr. Manindra Nayak, Consultant Surgical Oncologist, used a scarless, transanal technique that allowed removal of the tumour while preserving the rectum and avoiding external incisions.
The patient, Mr. Sitaram Gauda, 52, had previously visited other hospitals, where he was advised to undergo alternative, more invasive procedures for his rectal tumour. At Apollo Cancer Centre Bhubaneswar, a multidisciplinary team carefully evaluated key clinical criteria:
• Size of the lesion: The tumour was 2 cm x 2 cm.
• Location: Approximately 5.5 cm above the anal verge.
• Surgical time and inpatient duration: The procedure lasted just 20 minutes, and the patient stayed only three days in the hospital, promoting faster recovery and earlier discharge.
• Advanced surgical devices: ACC utilized the leading-edge GelPoint transanal port and a laparoscopic harmonic device for high-precision excision.
• Procedure specifications: Margins around the tumour were marked using an energy device. The tumour was excised with adequate margins, and the defect was closed using 3-0 PDS suture, ensuring safety and effectiveness.
• Selection criteria: This approach is ideal for select early-stage tumours and requires expert surgical hands.
• Hereditary factors: Family history and genetic risks were reviewed to determine personalized management.
