The robust initiative prioritises post-discharge care to reduce readmission rates and boost long-term recovery post-cardiac surgery.

In recent years, India has recorded an upsurge in the prevalence of sedentary lifestyles, unhealthy diets, and tobacco usage. The post-discharge cardiac care program reported a readmission rate of 3 in 100 amongst the enrolled patients, and a 90% increase in follow-up appointment compliance with the primary cardiac surgeon. Furthermore, the use of risk-stratification tools has helped identify patients with a high risk of mortality, paving the way for more targeted intervention.”
