  • Apollo Homecare Prioritises Post-Discharge Care for Cardiac Surgery Patients with Home Discharge Program

     Apollo Homecare, India’s leading homecare service provider has successfully completed the first phase of its one-of-its-kind Home Discharge Program for cardiac surgery patients. The robust initiative prioritises post-discharge care to reduce readmission rates and boost long-term recovery post-cardiac surgery.

    In recent years, India has recorded an upsurge in the prevalence of sedentary lifestyles, unhealthy diets, and tobacco usage. Talking about the same, Vishal Lathwal, CEO of Apollo Homecare said, “This unhealthy change has resulted in a substantial increase in cardiac diseases, which has led to approximately 3 lakh cardiac procedures being performed annually across 420+ cardiac centers. At the same time, the period from discharge to recovery remains critical for patients. Thus, to ensure expedited recovery and long-term well-being of patients, Apollo Homecare has taken the initiative to offer a Post-Discharge Care program to bolster the recovery process, and ensure patients receive top-notch quality care within the comfort of their homes.”

    Capitalising the focus on the period following discharge from the hospital, the new vertical facilitates real-time patient care by regularly monitoring the progress and identifying any potential risks that could affect the recovery process. In addition, patients get access to the expertise of expert doctors, specialised physiotherapists, and trained nurses at all times, ensuring the delivery of continuum care at home in a specialised vertice such as cardiology.

    Addressing the need for post-discharge care of cardiac surgery patients, this highly-effective program has been instrumental in facilitating healing, whilst also reducing the risk of complications to enhance the patient’s quality of life. In addition, it also offers a holistic outlook to recovery by promoting a seamless transition from hospital to home, which ensures the continuum of care post-discharge.

    Commenting on the success of the program, Dr. Sai Kumar of Apollo Homecare said, “The Post-Discharge Care Program received a positive response in Hyderabad that recorded over 900 enrollments of patients experiencing positive outcomes, along with reduced burden. The post-discharge cardiac care program reported a readmission rate of 3 in 100 amongst the enrolled patients, and a 90% increase in follow-up appointment compliance with the primary cardiac surgeon. Furthermore, the use of risk-stratification tools has helped identify patients with a high risk of mortality, paving the way for more targeted intervention.”

    The initial phase of the program has received positive feedback from the physicians and surgeons apart from the patients. With the positive results reported by the first phase of the ‘Home Discharge Program’, Apollo Homecare is looking forward to boosting its penetration in other cities, offering them the same set continuum of care after discharge.
